To register with this Practice permanently, please click the green circle register online button to the right hand side of your screen, complete and submit the form. You will also need to complete the patient health questionnaire below.
(Please note: You need to be over the age of 16 years old to complete this form on-line, please contact the Patient Liaison Team if you are trying to register a child under 16 year old).
If you are trying to register a child under 16 years old please note that a parent must be permanently registered with the Practice for the Practice to accept your application..
Once you have completed and submitted the registration form could you please submit a New Patient Questionnaire
Upon Receipt of the above submitted forms we will arrange a call back/video consultation to verify your identification.
We require to view the following:
- 3 forms of identification to confirm residency/identity.
(One form of identification should be photographic- for example passport, driving license, bus pass or identity card) if you do not have photographic identification we can accept your original birth certificate. The other 2 items need to show your current address and be no more than 3 months old (for example any household bills, bank statement or wage slip).
- Please note that documents may be verified by the Home Office.
On receipt and verification of you ID we will confirm your permanent registration in writing to you within 7 working days.
Should your application by unsuccessful you will still be able to be seen by a clinician for any immediate and necessary treatment you may require.
Giving Access to your Medical Records:
If you wish to give a family member/friend access to your medical records, or the ability to speak to a Clinician on your behalf, please speak to The Reception Team, who will be able to either email or print you a copy of the consent form for you to complete. This form will enable the person given access to book appointments, speak to a clinician on your behalf and obtain information needed from your record (eg test results).

When registering at the practice please ensure to tell us if you will a telephone translator when you speak to a GP or Nurse as this will enable your communication with the clinical staff much easier. We offer a range of telephone translation languages so there will be no need to worry.

All new patients registering with the Practice will be invited for a health screening assessment.
This is a valuable check-up as it can identify potential problems and introduces you to the Practice and to the services we can offer you and your family.
