COVID restrictions will end in many settings in England from Monday 19th July 2021. However, Public Health England's Infection Control Guidelines are set to remain in place for all NHS settings. Staff, Patients and Visitors will be asked to continue to, wear a face covering, use the intercom on arrival, sanitise hands and social distance when attending the surgery. Improved Access Appointments (Out of Hours Appointments): Our improved access appointments which are available at The Hub which is at The Riverside Surgery, Evesham, Monday to Friday from 6.30pm to 8.00pm and weekends from 8.00am to 12noon are now available for patients to book again. Welcome to Merstow Green Medical Practice, we are a GP surgery situated in the centre of Evesham and cover a wide range of the surrounding villages.

 .

We aim to provide all our patients with a high standard of personal, caring family medicine within the resources available to us and to deal with any problems you may have as quickly and efficiently as possible. We hope to use our Surgery website as a way of presenting all our patients with an up to date resource for all information relating to our Practice.  We will keep it current with any news, developments and details that are relevant to the Practice and our patients. As well as a source of information about our Practice, the staff and the services we provide, we hope you use the website as a useful resource with links to other reliable websites for health related information.  If there is topical health information we will also add this to the website under the latest news banner.

Merstow Green Medical Practice is also a GP Training Surgery. This means we will have GP's at different stages of their training gaining vital experience working within a GP surgery. Our current GP trainee is Dr Asghar and Dr Toh. Dr Kundra has now finished his training with the Surgery and has moved forward to a full time GP role with another GP surgery in the UK.

Our team includes 7 GP's, 2 Trainee GP's, 5 Nurses, 3 Health Care Assistants, 1 Phlebotomist and our in-house Practice Pharmacist and Dispensary Team (which is available to eligible patients) with a delivery service where appropriate. Alongside our Patient Liaison Team, Medical Secretaries and Management staff.

Should you wish to register at our Practice, whether new to the area or from another local Doctor's Surgery and you live within our Practice catchment area, please complete a registration form and submit, click on our Register Online button below. Alternatively please telephone a member of our Patient Liaison Team if you have any queries or wish to discuss further, they will be happy to help. Please also note that we need your correct contact information for all patients new and existing to help keep your records up-to-date. If you do not want your patient data shared with NHS Digital, you can register a Type 1 Opt-out with your GP practice by returning this form . You can register a Type 1 Opt-out at any time. You can also change your mind at any time and withdraw a Type 1 Opt-out.

Data sharing with NHS Digital will start on 1 September 2021.

If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can send the form by post or email to your GP practice or call 0300 3035678 for a form to be sent out to you. If you do not want NHS Digital to share your identifiable patient data (personally identifiable data in the diagram above) with anyone else for purposes beyond your own care, then you can also register a National Data Opt-out.
